This tool combines the historical Matzke creatinine-clearance relationship with an editable volume-of-distribution assumption. It can help anticipate a regimen’s concentration profile, but it cannot know the patient’s actual clearance or distribution before concentrations are measured.
- Not Bayesian forecasting and not a substitute for measured levels or a validated population model.
- Not intended for unstable renal function, dialysis or CRRT, ECMO, pregnancy, pediatrics, major burns, severe fluid shifts, or other markedly altered pharmacokinetics.
- First-dose levels are not steady-state levels; the calculator reports them separately.
