Intended population

Adults with stable serum creatinine when Cockcroft–Gault is the renal estimate required by the dosing evidence.

Limitations and exclusions

  • Not validated here for patients younger than 18 years.
  • Unreliable in acute or rapidly changing kidney function.
  • Not a substitute for measured creatinine clearance when measurement is clinically necessary.
  • Use caution in pregnancy, dialysis, severe muscle-mass extremes, amputation, cachexia, or unusual body size.
  • The 0.85 female coefficient is historical and does not represent contemporary gender identity.
  • Low serum creatinine values are not rounded automatically.
