This calculator requires two accurately timed post-distribution levels from the same steady-state interval, a stable dose and interval, and stable elimination. It is not valid for rapidly changing renal function, dialysis or other extracorporeal clearance, infusion overlap, undocumented doses, or samples taken during distribution.
- This is first-order one-compartment estimation, not Bayesian forecasting.
- The 400–600 AUC/MIC context applies to serious invasive MRSA infections with MIC assumed to be 1 mg/L; it is not a universal vancomycin target.
- The calculator will not recommend a replacement dose or interval.
