Clinical errors should be easy to report and visible when corrected.
Reports about a formula, source, unit, exclusion, interpretation, accessibility problem, or technical defect are treated as product-safety feedback.

Use the current instructions on the Contact page. Do not include patient names, dates of birth, medical-record numbers, images, or any other protected health information.
What to include
Name the tool, describe the concern, provide the expected behavior or controlling source when available, and include a reproducible example using fictional values only.
Safety triage
A report suggesting immediate patient risk is prioritized. Affected functionality may be restricted or removed while the source, code path, boundary behavior, and downstream wording are evaluated.
Correction process
Confirmed material corrections should update the relevant version, test coverage, date, evidence record, and public change description. Silent changes to clinical logic are not acceptable.
