Extended-interval protocols are not interchangeable.

Hartford interpretation requires gentamicin/tobramycin 7 mg/kg. The amikacin option is a separately labeled 15 mg/kg institutional adaptation that divides the measured level by two before plotting.

Do not use for endocarditis/synergy dosing, pregnancy, cystic fibrosis, major burns, ascites, spinal cord injury, dialysis/CRRT, unstable renal function, or other materially altered pharmacokinetics unless your protocol explicitly covers that population.
A random level outside 6–14 hours, or a dose that does not match the selected protocol, is not interpreted.
Near a nomogram boundary, the calculator intentionally moves to the longer interval.
