Extended-interval protocols are not interchangeable.

This tool implements named initial-regimen rules, not a universal aminoglycoside dose. Gentamicin/tobramycin use 7 mg/kg; amikacin uses 15 mg/kg and has a different lower creatinine-clearance boundary.

Do not use for endocarditis/synergy dosing, pregnancy, cystic fibrosis, major burns, ascites, spinal cord injury, dialysis/CRRT, unstable renal function, or other materially altered pharmacokinetics unless your protocol explicitly covers that population.
Actual weight is used unless it exceeds 120% of ideal body weight; then adjusted weight with a 0.4 factor is used.
The initial interval still requires confirmation with a precisely timed post-dose concentration and ongoing renal/toxicity monitoring.
