Gentamicin & Tobramycin PK
Calculate patient-specific first-order parameters from two measured levels, then optionally simulate—not select—a proposed conventional intermittent-infusion regimen.
Required model acknowledgement
This is not an extended-interval nomogram.
The calculation requires a stable conventional regimen at steady state and two accurately timed post-distribution levels from the same interval. It uses a one-compartment first-order model and does not establish whether a regimen or concentration target is appropriate.
- Do not use this workflow as a Hartford or other extended-interval nomogram.
- Changing renal function, dialysis or CRRT, burns, ascites, cystic fibrosis, pregnancy, major fluid shifts, uncertain dose history, or mistimed samples can invalidate the model.
- The optional regimen section projects concentrations from values you enter; it does not recommend a dose or interval.
About this tool
Gentamicin and Tobramycin PK Calculator
Use this gentamicin and tobramycin PK calculator for conventional two-level patient-specific pharmacokinetics and transparent regimen simulation.
The tool uses timed levels and a first-order model; it is distinct from extended-interval nomogram dosing. Validate sampling, infusion, dose history, renal stability, and local targets before use.
