IV Medication Compatibility with LR
Search common ED medications and antibiotics. Y-site evidence, drug-bag diluent support, concentrations, formulations, and contact time stay separate.
One question at a time
The quick chart answers “Can this share the running LR line?”
It does not answer whether LR may be used inside the medication bag. Switch to pharmacist detail whenever the product, concentration, formulation, or contact conditions are uncertain.

| CefepimeAntimicrobial | Check firstWhy this is uncertainA balanced-fluid study found short-contact Y-site compatibility with plain LR, while newer work found concentration- and time-dependent changes beyond 1 hour. Some labels list D5/LR—not necessarily plain LR—as an infusion fluid. Follow the exact product label and institutional database; do not extrapolate prolonged contact. Not established by cited Y-site study · Cefepime prescribing information · Cutshall et al. 2025 | Check the exact formulation, concentration, or local compatibility resource before connecting. |

What this tool is based on
This versioned record keeps the citations, equation or decision rule, units, test coverage, intended population, and exclusions attached to the tool.
How evidence review works →ED and inpatient clinicians checking a common IV medication against running Lactated Ringer's while retaining access to the exact product label and institutional IV compatibility database.
Searchable synthesis separating simulated Y-site physical compatibility from manufacturer-supported use of LR as an infusion diluent, with formulation, concentration, contact-time, and age exceptions shown in the result row.
Reference table only. Tested concentrations are displayed in each row; mg/mL and units/mL are not normalized across products.
Search and category/status filters, alphabetized nurse action and visual-matrix views, keyboard-accessible uncertainty popups, formulation-dependent Zosyn row, ceftriaxone calcium/neonatal warning, separation of Y-site and diluent claims, eight study incompatibilities, newer amiodarone evidence, one-page PDF integrity, and empty results.

Limitations and exclusions
- A compatible Y-site result does not establish LR as an acceptable reconstitution or infusion-bag diluent.
- Physical compatibility does not prove chemical stability, potency, sterility, or clinical safety.
- Results cannot be extrapolated to a different concentration, manufacturer, excipient package, temperature, mixing ratio, container, contact time, or balanced crystalloid.
- Ceftriaxone-calcium labeling overrides short-contact physical observations; neonatal restrictions are stricter than adult sequential-use instructions.
- Piperacillin/tazobactam compatibility is formulation dependent; confirm EDTA/edetate disodium on the actual product.
- Absence from the table means no conclusion, not compatibility.
- Blood-product coadministration and neonatal/pediatric procedures require the applicable product labeling and institutional policy.

Lactated Ringer's (LR) Compatibility Chart
Use this Lactated Ringer's compatibility chart as a rapid visual cross-reference, then open the detailed evidence view when a medication needs closer review.
Compatibility can depend on concentration, diluent, contact time, route configuration, and source conditions. Verify the exact preparation in a current compatibility resource and local policy before coadministration.
