Limitations and exclusions
- SOFA does not by itself diagnose sepsis.
- Baseline score and change over time must not be assumed automatically.
- Vasoactive dose, respiratory-support status, urine-output interval, and laboratory units must match the published definitions.
- Sedation, paralysis, chronic organ dysfunction, and other confounders can distort individual components.
